Predixion Software is demonstrating Predixion Readmission Insight(TM), a preventable readmission healthcare solution built on Predixion’s predictive analytics platform at the HIMSS12 Annual Conference & Exhibition where Predixion is exhibiting in booth #9308.
Predixion Readmission Insight was designed in collaboration with provider organizations. The solution identifies patients at risk of readmission before they leave the hospital, allowing for more tailored care while admitted and post discharge to assure a more successful care outcome. Predixion Software models have accurately predicted up to 86 percent of readmissions which would result in better patient outcomes and true ROI for the provider in recent evaluations.
Predixion Software Vice President of Healthcare Michael Hollenbeck and emids Vice President, Enterprise Services and Business Intelligence Rahul Ghate will co-present “Managing Preventable Hospital Readmissions via Predictive Modeling” in the HIMSS12 Knowledge Center #13247 today from 2:15 – 3:00pm PT. In the session, attendees will learn that with Medicare’s payment penalties for hospital readmissions fast approaching, predictive modeling is being applied to reduce preventable hospital readmissions by risk-scoring patients’ likelihood to readmit at or near the time of admission, along with identifying key drivers of readmissions within specified patient populations.
Hollenbeck will also participate in a HIMSS12 discussion “ACO Analytics and Informatics for Mummies: Avoid Getting Wrapped Up in Your Data!” on February 23 from 11:30am-1:00pm PT. He will be joined by a panel of leading payer, provider, and product executives as they discuss how they have leveraged data assets to build a solid foundation for an ACO.
